  2. All communication about concerns, variations to the Agreement, or other matters around the Agreement, should be directed to [email protected]
  3. Any revision to these arrangements will be agreed between the client and Blue Knot Foundation and confirmed with an amended Service Agreement Letter.

  5. The client will be responsible for liaising directly with their supervisor to establish days and times for the above services per the agreed schedule.
  1. The client acknowledges this is an external supervision service for members of your team and not a counselling or crisis intervention service. All matters of a personal nature for any individual need to be managed separately from the supervision sessions (e.g., Line Management, HR, or an EAP service intervention). Blue Knot Foundation will not be liable for the personal health and wellbeing of participants resulting from either personal and/or professional workplace matters.
  2. The supervisor providing the supervision agreed to in this document will not provide any direct clinical care to clients or provide supervision in a way that involves them having clinical contact with clients.

Cancellations and Rescheduling

b         Notice of cancellation must be provided by the party cancelling to the other party in writing, e.g., by email.

c.        If the client provides less than 2 days’ notice, the session will either need to continue as planned or be forfeited. If a session is forfeited due to cancellation or non-attendance this session is non-refundable. A make-up session will   be at the standard additional cost.

d.      If the supervisor provides less than 2 days’ notice, the session will be rescheduled to occur as soon as is practicable for the supervisor and client.

9. Postponement of service agreement: Blue Knot Foundation will allow postponement of the agreed services if exceptional circumstances occur which prevent the service from proceeding, and at Blue Knot’s sole discretion. 10. Blue Knot reserves the right to terminate this agreement if the Supervisor has been placed at risk by either the organisation or the supervisees.

11. While this service agreement is in place or at any time after this service agreement ends for any reason, all parties must not, except as required by law while providing the service, use, disclose, divulge, communicate, or otherwise place at the disposal of any third party in any form or by any means, and must keep in the strictest confidence, any Confidential Information acquired. This clause will survive despite the termination of this service agreement for any reason.

13. Each party must comply with all Privacy Law in relation to Personal Information, whether or not the party is an organisation or entity bound by the Privacy Act 1988, the Information Privacy Act 2014 (ACT) or the Privacy and Personal Information Protection Act 1998 (NSW), whichever is relevant.

14. All precautions must be taken to prevent unauthorised access, use or disclosure of confidential information.

15. Blue Knot Foundation requires that no service delivered by Blue Knot Foundation is recorded, in part or whole, or in any way. Nor any screenshots be taken during the session.

16. Blue Knot Foundation works within the bounds of mandatory reporting. If any risk issues arise during the sessions, Blue Knot Foundation will manage these as appropriate with the supervisor and organisation in the first instance. Further action may occur after these discussions.

17. Should any unforeseen circumstances occur, Blue Knot Foundation reserves the right to amend the service arrangement and will work with the client to provide the most suitable option.

18. Should the identified supervisor not be available to continue the service, Blue Knot Foundation will provide the client with alternative supervisors.
